New Athletes -

Concussion Baseline Assessment via SWAY Medical
Physicals – must be dated after June 1, 2026 and by a M.D. or D.O. or NP/APRN (chiropractors will not be accepted)
- Link to Physical Form
- Upload physical form to Sportsware under the "Attachments" tab
Sickle Cell Trait Status
- Upload obtained / previous test results in Sportsware under the "Attachments" tab
- If unable to get the test prior to arriving on campus, our Health Services Dept. is able to provide the test for a $25 fee
