BVU Sports Medicine Checklist

New Athletes -
  • Concussion Baseline Assessment via SWAY Medical  
  • Physicals – must be dated after June 1, 2026 and by a M.D. or D.O. (chiropractors will not be accepted)
  • Sickle Cell Trait Status  
    • Upload obtained test results
    • If unable to get the test prior to arriving on campus, our Health Services Dept. is able to provide the test for a $25 fee
